Technology Helps Facilitate Right Care/Right Time

The importance of right care/right time in improving patient care and moving healthcare in the right direction cannot be overstated. And when I talk to home health agency executives, they tell me one thing has become completely clear: the key is putting the right tools into clinicians’ hands. These include workflow tools, like those included in home health software, that help them complete documentation more quickly and robust clinical decision-support tools.
For example, they need to be able to quickly gather the necessary information from their referral partners and get an initial visit assigned to a staff member. Next, they need to be guided through the assessment and care planning process, with best-practices and evidence-based practices clearly outlined. It goes without saying that those practices must be based on validated professional research, guidelines and protocols.
It’s important for the tools to provide visual clues during the referral, intake and clinical documentation process to indicate the status of the work that remains to be done. Clinicians also need tools that automatically create outputs such as physician orders and printable assessment tools.
With proper tools, clinicians can focus on keeping patients – even those with more than one chronic condition – in their homes with improved outcomes. These tools have others benefits, such as shortening the revenue cycle and improving documentation compliance. The latter is critically important, with CMS scrutinizing clinical documentation more closely than ever.
